This is an attestation for inoculation of Cowpox as a protection against Smallpox.
See here.

It says:
"Cowpox Inoculation Attestation
Lilly Petrine born in Bergen of the parents Shoemaker Axel Martin Olsen and Margrete Mathilde Olsen and living in Bergen being 1 year and 11 months old is by me the undersigned in the year 1906 June 20 inoculated with Cowpox. By detailed control between the 7th and 9th day after inoculation I have found that all the signs show them to be the real Cowpox; they were complete and unhurt, filled with a clear fluid, in the middle depressed and surrounded by a red circle;
Lilly Petrine has therefore had the real Cowpox which protect against Smallpox, which is hereby witnessed by
Bergen June 27 1906
Signature"

In 1900 the family were living in Madsensmuget 8 an had come to Bergen approx 1897.
